Phyllis T. Jakubczak
March 20, 1925 — November 7, 2023
Phyllis T. Jakubczak, (Nee: DeLucia) 98 of South Amboy entered into eternal rest on Monday, November 6, 2023 at the Venetian Care Center.
Phyllis was born in South Amboy, daughter of the late Antonio and Maria DeLucia, where she lived most of her life. She was previously employed by South Amboy Memorial Hospital and enjoyed spending time with her family. Phyllis loved watching the New York Mets.
She is predeceased by her husband Adolph Jakubczak in 1980, her daughter Michelle O’Toole in 2023, her sister Mary DeLucia and her brothers Pip, Tony, Sammy, Al and Frankie DeLucia.
Phyllis is survived by her loving children Janice Izworski and her husband Kevin of Tennessee, David Jakubczak and his wife Nancy of South Amboy and her son-in-law William O'Toole of Hazlet, Her dear 8 grandchildren and 10 great grandchildren. Her siblings John DeLucia and his wife CarrieAnna of Morgan section of Sayreville, and Ann Kuchta and her husband Joe of Sayreville, and her sister-in-law Gladys Delucia of Sayreville. And many loving nieces and nephews.
A 10 am funeral mass is scheduled on Saturday, November 11, 2023 at Saint Mary Church, South Amboy, NJ.
